SERVICES TO BE PROVIDED
The Facilitator provides the following medical facilitation services to the Patient:
- Coordinating consultations with healthcare professionals.
- Facilitating medical procedures and treatments.
- Assisting with travel arrangements, including accommodation and transportation.
- Providing necessary documentation for medical procedures and travel.
The Patient should acknowledge and understand that the Facilitator is not a medical professional and is solely responsible for coordinating logistical aspects of medical services.
FACILITATOR'S RESPONSIBILITIES:
The Facilitator will use reasonable efforts to:
- Facilitate appointments with healthcare providers for consultations, examinations, and treatments.
- Provide information regarding medical procedures, potential risks, and benefits.
- Coordinate travel and accommodation arrangements based on the Patient’s preferences.
- Assist in obtaining necessary medical records and documents.
The Facilitator will communicate promptly with the Patient about any changes or updates to the medical and travel schedule.
PATIENT'S RESPONSIBILITIES:
- The Patient should agree to provide accurate and complete medical information to the Facilitator.
- The Patient is responsible for obtaining any necessary visas, travel insurance, and complying with all relevant laws and regulations. The hospital/medical facility provider will supply and help in all the supportive documents to obtain visa, necessary permission and compliances.
CONFIDENTIALITY:
Both parties agree to maintain the confidentiality of all medical and personal information shared during the facilitation process.
GOVERNING LAW:
This Agreement shall be governed by and construed in accordance with the laws of India with jurisdiction of Mumbai Maharashtra.
